The
U.S. Administration on Aging sponsors this Exchange to make information and resources available to states and community organizations. AoA is committed to supporting states' efforts to develop and sustain a person-centered, self-directed national long term care system. This system should effectively assist consumers with identifying and accessing a range of home and community based resources that maintain individual independence. This website provides a forum to allow a diverse community of stakeholders involved in making changes to their long term care system to exchange ideas, knowledge, and best practices.
